- Certified Birth Certificate
- Social Security card
- MS Immunization Form
(Form 121)
- 2 Current Utility Bills (proof of residency)
- One mortgage document (Lease/rental agreement, Warranty Deed,
Settlement Statement, or Homestead Exemption receipt)
